Medicines Optimisation Pharmacy Technician

37.5 hours per week - fixed term for nine months

We are recruiting an experienced pharmacy technician to support medicines optimisation services at Fieldhead Hospital, Wakefield, on a fixed-term contract to cover maternity leave. The role requires enthusiasm, effective communication, teamwork and the ability to work independently with integrity and autonomy.

Standard working pattern is 9:00–17:00 with weekend commitment. This post is offered as full time, 5 days per week. The role will be based at Fieldhead Hospital in Wakefield with some occasional cross-cover at other Pharmacy bases. Flexible working can be discussed at appointment.

Main duties of the job

The successful candidate will work with speciality pharmacists and the MDT to coordinate all aspects of the patient Medicines related journey, ensuring safe, seamless and integrated transfer of care.

Key result areas include:

  • Supporting clinical teams with the supply, storage and management of medicines within inpatient areas.
  • Completing medicines history/reconciliation on admission.
  • Providing information to patients and carers about their medicines and supporting adherence and understanding.
  • Working with clinical teams to plan and support timely leave and discharge, utilising near patient dispensing where appropriate.
  • Supporting transfer of care to other settings and collaborating with primary care teams on discharge planning.
  • Accuracy checking medication for leave/discharge.
  • Supporting the training and development of trainees.
  • Participating in the weekend and bank holiday rota.

Job responsibilities

JOB SUMMARY

This job description outlines the main duties of the post. The postholder will undertake other duties commensurate with the grade as directed.

  • Provide ward pharmacy service to allocated units within the SWYPFT remit.
  • Provide technical support to the mental health pharmacy services including checking patients' own medication on admission, dispensing and supplying medication to specific patients.
  • Coordinate and manage day-to-day workload in the dispensary.
  • Ensure staff are up to date on dispensary competencies.
  • Provide immediate line management support for allocated staff (pharmacy technicians band 4, pharmacy assistants or student technicians).
  • Maintain SOPs for pharmacy stock control systems (PSC) and dispensary processes, updating as appropriate and ensuring alignment with national and local guidance.
  • Investigate and correct stock discrepancies as per SOPs.
  • Maintain drug and label files on the PSC following SOPs.
  • Be the point of contact for queries relating to PSC usage.
  • Process and match month-end paperwork from wholesalers; investigate and correct mistakes following SOPs.
  • Extract data from PSC using SOPs and upload data required for national submissions.
  • Assist with business continuity planning and PSC testing.
  • Provide day-to-day line management for allocated dispensary staff.
  • Accurately check completed prescriptions and maintain competence to work across the trust.

The content of this post will be reviewed in consultation with the postholder as needed, in line with service developments.

KEY RESULT AREAS:

  • Improve access to medicines for allocated units, ensuring medicines are available for leave, discharge and self-medication in a timely manner.
  • Participate in Medicines Management Service at ward level, maintaining competencies in all aspects.
