The Care Coordinator is responsible for coordinating the day-to-day delivery of high-quality domiciliary care services, ensuring that service users receive safe, person-centred, and responsive care. The post holder will effectively manage staff rotas, allocate care visits, support care workers, liaise with clients and healthcare professionals, and ensure compliance with CQC Fundamental Standards and company policies.
The Care Coordinator plays a key role in maintaining continuity of care, promoting positive outcomes for service users, and supporting the Registered Manager in achieving excellent service quality.
Key Responsibilities- Allocate care workers based on client needs, location, experience, skills, and continuity of care
- Respond promptly to changes in service users' needs and adjust care schedules accordingly
- Maintain regular communication with care workers regarding schedules and service updates
- Assist with onboarding new care staff, including introductions to service users where appropriate
- Support compliance with CQC fundamental standards and associated health and social care legislation
- Report incidents, accidents, errors, and safeguarding concerns promptly
- Liaise with social workers, community and district nurses, GPs, commissioners and family members of service users.
- Experience working within domiciliary care
- Excellent organisational and time management skills
- Strong communication and interpersonal skills
- Leadership and team-working skills
- Ability to prioritise workload and work under pressure
- Knowledge of safeguarding adults
- Knowledge of CQC fundamental standards
- Full UK driving licence and access to a vehicle
- Experience using care scheduling software (e.g. Birdie, Care Planner, PASS, or similar)
- Experience participating in an on-call rota
- NVQ Level 3 in Health & Social Care (or equivalent)
- Mandatory Healthcare Training
- Enhanced DBS (on update service)
- Business Insurance (for car) (preferred)
- Office-based with regular travel within the community
