An exciting opportunity has arisen for a Staff Nurse to join our growing team on a full time (34.5 or 37.5 hours available). Wakefield Hospice is looking for those of you who have the passion and drive to deliver high quality specialist palliative care in our inpatient unit, have effective communication skills and at least 12 months UK post‑registration experience in working in community care settings.
You will be provided with a full induction programme, a buddy/mentor, and general support to allow you to keep developing your skills and knowledge in specialist palliative care whilst working in a high quality clinical environment.
The successful candidate must demonstrate compassion, commitment, caring, competence, quality and safety in delivering person centred care.
Main duties of the jobWakefield Hospice provides specialist palliative and end of life services 24 hours a day, 7 days a week. You will be required to work a variety of shift patterns across 24hrs, including rotation to day and night shifts (and on occasion to support the Day Therapy Services) to:
Deliver high quality Specialist Palliative Care and End of Life Nursing as part of the Inpatient Unit multi professional team.
To assess, plan, implement and evaluate care for a named group of patients ensuring a holistic approach that is sensitive and responsive to patient and family's needs.
To demonstrate compassion, commitment, caring, competence, quality and safety in delivering person centred care.
To be responsible for the coordination and smooth running of a shift when required, ensuring it is well organised and safe.
As a registered, accountable and autonomous nurse act in the best interests of patients at all times, taking personal responsibility for own actions and decisions.

Wakefield Hospice is a purpose-built specialist palliative care unit that opened in April 1990. Currently undergoing a period of refurbishment, it will have 16 single ensuite bedrooms. The hospice also has a dementia care service, bereavement support and education. Over the years the service has developed initially into a day therapy unit, and ultimately into a drop‑In centre providing therapies and wellbeing, information and care for patients, carers and bereaved families.
Jobresponsibilities
Clinical
Assess, plan, implement and evaluate person centred nursing care for a named group of patients in compliance with hospice policy and protocols.
Undertake holistic assessment of patients, families and carers, including their emotional, physical, spiritual, practical, welfare rights and as appropriate pre and post bereavement needs. Ensuring that patients, their families and carers are involved in their plans of care.
Actively support and plan the discharge planning and process.
To develop, update, monitor and evaluate patient care plans ensuring they are person centred and support individual needs.
To demonstrate the skills and knowledge that enable the delivery of basic and complex specialist nursing care interventions that are responsive to individual need.
Have knowledge of common symptoms, anticipate change and evaluate interventions.
To communicate professionally, sensitively and effectively with patients, carers, families, the multi professional team and all staff within the hospice.
To deliver concise and accurate patient presentation in daily hand overs and to attend and participate in the weekly MDT meeting in the absence of senior nursing team if requested.
