We are delighted to offer an exciting opportunity to join our team as a Neonatal Outreach Nurse at Mid Yorkshire Teaching NHS Trust. This is a fixed term post - 2 years contract.

The primary aim of the role is to ensure that a high-quality Neonatal outreach care is delivered to neonates and their families.

The Nurse will plan, deliver, and evaluate the care of Neonate and their families in the neonatal unit and home setting.

As a Nurse, all activities provide opportunities for acting as a role model for staff.

The post holder will be responsible for ensuring the planning, delivery and evaluation of care to meet the personalised health and wellbeing care needs of patients, in line with the Trust’s Values and Nursing & Midwifery Professional Framework.

This will be achieved through the core competencies/ dimensions of the role, clinical practice, and acting as the patient advocate.

• To assist staff, based on the Neonatal Unit and Transitional Care Unit, with early and appropriate discharge planning in accordance with the criteria for neonatal outreach care.
• To provide a seamless timely discharge from the hospital to the home environment.
• To provide on-going holistic care within the community setting for vulnerable neonates and their families.
• To liaise with members of the primary health care team and other health care professionals to ensure the provision of appropriate seamless, on-going care.
• To work effectively and efficiently as an Outreach team member as well as independently in the planning, assessment, implementation and evaluation of neonatal community care.
• To plan, time manage and initiate a weekly schedule based on workload requirements, which will remain flexible, thus ensuring that individual family needs are met.
• Prepare and receive reports from health care professionals, ensuring good communication and working links with all others.
• Maintain accurate, legible and legal clinical records relevant to the care of the neonate in the community.
• Participate in nurse led clinics when appropriate.
• Develop enhanced skills in neonatal resuscitation with attendance at Trust and National Newborn Life Support courses.

• Registered Nurse/ Midwife / BSC/1st degree (health related) or equivalent
• Qualification/training and experience in specialty
• Evidence of ongoing professional development
• Neonatal Life support NLS

• Significant proven Neonatal qualification and intensive care experience
• Knowledge and Awareness
