Position SummaryProvide case management services to individuals with intellectual or developmental disabilities. Establish positive working relationships, develop person‑centered plans, and ensure compliance with state mandates. Monitor service quality and collaborate with external stakeholders.
SCI Team Member ExpectationsFollow the organization’s mission, core values, policies, and HIPAA confidentiality. Participate in leadership meetings, training, and maintain compliance with federal, state, and local employment laws.
Essential Duties- Advocate for individuals and educate them on self‑determination.
- Collect eligibility information and recommend priority to DDA services.
- Conduct comprehensive assessments and develop person‑centered plans.
- Coordinate resources, provide referrals, and assist with transitions to community settings.
- Monitor and update plans, conduct on‑site visits, and submit required paperwork (e.g., RECON, AIR).
- Maintain HIPAA record confidentiality and use tracking/time‑management tools.
- Build partnerships and collaborate with others.
- Communicate effectively across diverse audiences.
- Deliver customer‑centric solutions and adapt to changing situations.
- Demonstrate trust, integrity, and resourcefulness.
- Plan and prioritize work to meet organizational goals.
- Bachelor’s degree in a human services field; or
- Bachelor’s in a non‑human services field with two years of experience in a human services setting.
- Experience working with people with developmental disabilities.
- Standard office environment with >95% stationary time; occasional light lifting (10‑25 lbs).
- Requires computer use and, occasionally, vehicle operation.
