Youth Care Coordinator

Inner Sourced Solutions Inc., has an immediate opening for a part-time, PRN Youth Care Coordinator for 30 days. Work with youth (ages 6-22) and their caregivers to identify needs and goals that will strengthen the family and the youth's future. Areas of focus include education, mental health, physical health, economic needs, and positive social development. While face-to-face visits occur twice monthly, you will actively work on behalf of each client and family weekly through research, resource coordination, collateral contacts, and system navigation.

Assignment Duration:
This position is a short term coverage assignment not to exceed five weeks, providing direct coverage for a Youth Care Coordinator on approved leave

Hours:

As needed to meet coverage requirements for the assigned caseload; no minimum or maximum weekly hours are guaranteed for the duration of the assignment Compensation: $40 per hour. Client-related work is paid on a fee-for-service basis: time is payable only when it corresponds to a documented task or encounter performed with or on behalf of an assigned client, evidenced by a corresponding encounter note in InSync.

There is no guaranteed number of hours, and time is not payable based on availability or a fixed schedule. Supervision and other administrative time assigned by the supervisor are paid separately at the applicable hourly rate. Mileage is reimbursed at the current IRS standard mileage rate for round-trip travel from the office to the client's home and back.

Key Responsibilities:

• Providing in-person case management services to children and adolescents based on authorized level of care, including a minimum of two face-to-face visits per month with ongoing weekly work on behalf of clients and families as needed
• Conducting twice-monthly face-to-face visits in homes, schools, or community settings
• Assessing needs, researching and collaborating/connecting clients/families with resources, services and support systems
• Assisting clients/families to navigate healthcare and education systems and understand their options
• Connecting participants to programs and services based on identified needs
• Marketing the Youth Care Coordination through outreach to schools, pediatrician offices, and other referral sources
• Completing required documentation and progress notes within a timely manner
• Participate in bi-weekly supervision Work Setting:
Not location restricted. Given the limited scope and duration of this assignment, this role is not held to the standard on-site or in-community requirement that applies to standing Youth Care Coordinator positions. Completion of work is verified through documentation of client contact and completion of assigned tasks, not through physical presence at the office or in the field. What You Should Know

• Work varies in pace; you must be self-directed enough to stay productive during slower periods and organized enough to manage multiple priorities during busier times
