OT Occupational Therapist

Overview

Home-Based Acute Care Occupational Therapist with the joint venture between Providence and Compassus. In this role, you’ll bring inpatient-level therapy into the home, providing comprehensive evaluations, personalized treatment plans, and thoughtful discharge planning. Work independently and as part of a multidisciplinary care team to support patients through critical transitions with compassion and clinical excellence.

• Deliver comprehensive therapy services including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluate clients' physical, cognitive, psychological, and perceptual functions to enable safe and effective functioning at home.
• Develop individualized plans of care in collaboration with patients and families.
• Identify and make referrals related to medical, social, and emotional factors impacting care.
• Educate and counsel clients and families on treatment plans, safety concerns, and therapy goals within scope of practice.
• Adapt equipment and therapy techniques to the home environment to promote safety and optimal function.
• Independently manage caseload, schedule and coordinate home visits, and collaborate with the interdisciplinary team.
• Organize a weekly schedule balancing client visits and required team meetings.
• Supervise and instruct Home Health Aides on care plans, including functional skills training and exercise programs.
• Provide supervision and case management for clients served by Certified Occupational Therapy Assistants (COTAs), including completing every fifth visit.
• Deliver in-services to staff on new treatments or techniques.
• Complete documentation on the same day as visits, in line with agency guidelines and reimbursement requirements.
• Support onboarding of new team members by introducing them to clinical practices in home health settings.

• WA:
  Required - Bachelor's degree in Occupational Therapy or Master's degree in Occupational Therapy
• WA:
  Preferred - 1 year of Occupational Therapy experience with adults in a home care setting

• WA:
  Required - Current, unencumbered license as an Occupational Therapist (State of Employment)
• WA:
  Required within 30 days of hire - National Provider BLS - American Heart Association
• WA Hospice:
  Required within 30 days of hire - National Provider Identifier (NPI) number

The role requires standing, walking, manual dexterity, and the ability to lift and move objects up to 50 pounds. Visual acuity for close and distance vision, color, peripheral vision, depth perception, and adjustable focus are required. Exposure to bodily fluids and infectious conditions may occur. Routine use of standard medical equipment is required. Accommodations can be made for team members with disabilities.

Base pay range: $41.65/hr - $64.65/hr offering a $15,000 sign-on bonus for full-time OT hires (limited time).
