• Synthesize and execute individual physical therapy treatment plans as directed by the Rehabilitation Services Manager.
• Collaborate with a multidisciplinary care team—nurses, other clinicians, and providers—to optimize outcomes.
• Document progress notes and update treatment plans according to medical protocols and state regulations.
• Maintain accurate and up‑to‑date patient records in the electronic health system.

• Education and credentials meeting the licensure requirements for a Physical Therapist.
• Washington Physical Therapist License upon hire.
• National Provider BLS certification from the American Heart Association upon hire.

• Minimum 1 year of physical therapy clinical experience.
