DSHS HCLA Roads to Community Living Case Resource Manager
State of Washington – Vancouver, WA (hybrid work model). The Home & Community Living Administration (HCLA) seeks a Case Resource Manager to provide specialized case management and resource assistance for individuals with developmental disabilities and their families.
Responsibilities- Identify client support needs and authorize service payments.
- Schedule and conduct home visits, reviewing living arrangements.
- Facilitate goal-planning discussions with clients and families.
- Complete and revise assessments and service plans in CARE.
- Determine financial and functional eligibility for programs.
- Obtain written approvals and complete authorizations in Provider One.
- Monitor client progress and update plans.
- Request Waiver Enrollment, Exceptions, and Prior Approvals.
- Coordinate interpretation services when needed.
- Make referrals to appropriate agencies during crises.
- Maintain complete case files.
- Represent HCLA at meetings and provide transition supports.
- Professional, clear, grammatically correct correspondence.
- Organized work environment and prioritization skills.
- Basic computer skills: email, word processing, spreadsheets, CARE system.
- Capacity to safeguard confidential client information.
- Cultural sensitivity and collaboration skills.
- Ability to work independently and in a fast‑paced environment.
- Experience determining eligibility for services (interviews, rule interpretation).
- Knowledge of RCWs, WACs, policies related to Developmental Disabilities Administration.
- Certified interpreter status increases pay by 5% (not required).
Either a Bachelor's degree in social sciences, social services, human services, or allied field with either:
- 2 years experience providing services to individuals with intellectual or developmental disabilities; or
- 6 years experience; or
- 12 months completed as a Case Resource Manager Trainee; or
- Equivalent experience or education.
