About the Role
Are you looking to develop your leadership skills within a community setting? We have a fantastic opportunity for a Nurse to join our dynamic management team as a Palliative care Lead at Wallingford Community Hospital on St Leonard's Ward.
If successful, you will be expected to lead and support the registered and unregistered nursing staff in their professional development and promote best practices in palliative and End of Life Care. You will be responsible for delivering high-quality, patient-focused care to palliative and end-of-life patients and provide holistic support to their families.
You will work in partnership with key members of the Multidisciplinary Team in the hospital and external organisations such as the Sue Ryder team to promote and facilitate effective patient care and discharge.
As a palliative care lead, you will influence the development and implementation of policies, protocols and guidelines relevant to the ward area. As part of the management team it is our aim to improve the quality of care received and the experience of patients and their carers, through the implementation of initiatives which reflect the National Service Framework for Older People.
Face to Face Interviews are only available as a site visit and is required.
The post holder will be required to maintain their clinical expert practice, whilst promoting and creating a wider vision for palliative and EOLC.
The post holder will support staff learning and development across the trust in palliative and EOLC to provide patient and family centred care.
