Are you an experienced Palliative Care Nurse, Hospice Nurse or Community Nurse looking to take the next step in your career?
This is an opportunity to join a specialist palliative care service, earning up to 45,129, with full or part-time hours available, excellent benefits and fully supported Non-Medical Prescribing training.
You'll provide specialist care to patients with life-limiting illnesses across Berkshire West and South Oxfordshire, working autonomously while being supported by an experienced multidisciplinary team.
The Role As a Senior Palliative Care Nurse, you'll manage patients with complex palliative and end-of-life care needs, providing specialist assessment, symptom management and clinical support.
You'll be responsible for:
- Completing holistic assessments and developing individualised care plans
- Managing complex symptoms and supporting patients approaching end of life
- Providing specialist advice to patients, families, carers and healthcare professionals
- Supporting telephone triage and responding to clinical enquiries
- Working closely with GPs, community teams and the wider multidisciplinary team
- Advocating for patients and ensuring compassionate, person-centred care
- Maintaining accurate and high-quality clinical documentation
- Active UK NMC registration as an RGN/Adult Nurse
- Previous palliative or end-of-life care experience
- Strong clinical assessment and decision-making skills
- Excellent communication skills
- A full UK driving licence and access to your own vehicle
- The confidence to work autonomously within a specialist multidisciplinary team
