In today’s complex medical environment patients need dedicated primary care physicians to monitor and supervise their care. Our doctors have deep roots in the New Haven area; many having practiced here for over 20 years. We know the local medical community, and when necessary, we refer patients to the best specialists in the area. We admit to Yale‑New Haven Hospital and The Hospital of Saint Raphael.

Responsibilities
  • Evaluate patients, and treat acute and chronic illnesses
  • Orders, performs and interprets diagnostic tests
  • Involves patients and their families in care and treatment decisions
  • Creates treatment plans and completes orders in patient’s charts
  • Collaborates on improvements in care delivery and service with staff, other physicians and management
  • Initiates appropriate specialty referrals
  • Provides clinical information to the interdisciplinary team in a clear and timely manner
  • Ensures that proper patient documentation is timely, accurate and complete
  • Works with practice management to ensure proper compliance with billing, coding and CPT code assignment
  • Achieves and maintains optimal patient access
