We are seeking a full‑time RN clinical case manager with at least one year of home‑care experience and the ability to proficiently complete OASIS forms. The role requires a positive attitude, proficiency, and a warm heart. The agency offers top‑of‑industry rates.

• RN license (required)
• 1 year of home‑care experience
• Proficiency in completing OASIS forms
• Bachelor’s degree preferred
• English proficiency (required)

• Monday to Friday
• Weekend On‑Call rotation

• Mandated vaccination, PPE usage
