Caregiver / Personal Attendant / Direct Support Professional (DSP)

• Provide personal care assistance and companionship to your assigned client.
• Promote your client’s quality of life through meaningful activity and/or conversation.
• Provide light housekeeping and meal prep; transportation as appropriate.
• Follow care plans/instructions; communicate all changes and concerns with’avez Sage Home Care Manager.
• Complete an electronic daily shift report.
• Comply with Sage Eldercare’s Policies and Procedures and all applicable laws and regulations.

• Must have at least one year of experience working with a Home Care agency.
• Must have experience working with dementia clients.
• Must maintain a valid driver’s license and meet the organization’s auto‑insurance requirements. Occ rotten transport of client(s) is required.
• Must be passionate about working with seniors to improve their quality of life.

• Bronze Pay Rate – 4‑6 hours: $34.50/hr
• Silver Pay Rate – 6‑11 hours: $26.72/hr
• Overtime (12+ hours): $40.08/hr
• Overtime (12+ hours): $22.97/hr (alternative rate for 12‑hour shift)
