Position SummaryCOMHAR's Long Term Structured Residences (LTSR) are located in Northampton County. This position is in Walnutport, PA. These therapeutic programs provide 24/7 residential and intensive treatment supports for adults with a variety of psychiatric and health‑related needs. Services include individual and group therapies, psychiatric evaluation, crisis intervention, peer support, psychiatric rehabilitation, and medication management. The multidisciplinary team includes a psychiatrist, mental health professionals, licensed nurses, peer specialists, and direct‑care staff.
Services are delivered on‑site and in natural community settings to maximize learning opportunities. The goal is to help residents develop skills, increase independence, and stabilize psychiatric and physical health symptoms toward less structured settings.
- The Licensed Practical Nurse works from a recovery framework within the team, providing health and wellness care, medication education, group psycho‑education, therapeutic interventions (evidence‑based practices preferred), cultural competence and trauma‑informed care to help individuals obtain recovery goals.
- Assists residents who cannot self‑administer medication and supervises self‑administered medication when indicated. Injections are provided when prescribed. Participates in residents’ medication education, helping those who cannot self‑administer become able to do so.
- Participates in multi‑disciplinary treatment team meetings as assigned.
- Participates in Quality Improvement Program as assigned.
- Reports all crises and unusual incidents to Nurse Manager and LTSR Director immediately if indicated, otherwise within 24 hours; reports psychiatric and medical crises to Psychiatrist immediately.
- Complies with all internal and external record‑keeping requirements and regulations.
- Observes Universal Precautions in medications, needle disposal, and waste handling.
- Provides direct service to residents, including assistance with personal hygiene, bathing/showering, and other daily living tasks as needed.
- Leads activities with residents—educational, social, and leisure programs—eliciting residents’ input; participates in community integration activities as needed.
- Documents regularly and accurately in clinical chart and other designated forms.
- Attends all meetings and training sessions as assigned.
- Reports non‑emergent shift and personnel problems to Nurse Manager and/or LTSR Director promptly.
- When a staff member is absent, secures appropriate coverage via existing team members or approved temporary staff organizations.
- Adheres to all COMHAR policies and procedures without unauthorized exception.
Regularly required to talk or hear and accurately interpret speech and sounds; frequently required to stand, walk, climb and descend stairs; required to use hands and arms to reach, hold and carry objects; required to sit, stand and maintain balance; occasionally lift and/or move up to 150 pounds. Vision requirements include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus for reading, driving, general observation, and safety.

- LPN + at least two (2) years mental‑health or related experience (preferred).
- BS in Nursing + at least one (1) year of mental‑health or related experience (preferred).
- Must have current nursing license required
. - Valid driver’s license required
. - Must have own transportation.
- Valid PA State Criminal, FBI (for residents outside PA), and Child Abuse clearances.
- ACT 31 required
. - CPR with AED & First Aid certification required
. - Forensic experience preferred.
- Position requires occasional lifting, transferring of individuals averaging 150 pounds.
- Physical examination including PPD and Hep
