LPN – Home Health Nurse

We are seeking a compassionate and dedicated LPN to join our team. We offer flexible scheduling, weekday positions with low frequency weekend rotation and a holiday requirement of 2 holidays per year. You will be an integral member of the clinical team and will collaborate with other multi-disciplinary members to meet the needs of the patient.

• Walpole and 40 surrounding towns
• Full-time and Part-time positions available

• Provide patient revisits in accordance with the plan of care established with the patient’s physician.
• Ability to function independently as a clinician in the community.
• Comfortable with using an EMR.
• Great critical thinking skills and ability to triage patients and problems.

• Current LPN License from MA Board of Nursing.
• Two years’ experience in delivering nursing care.
• One year of home health care experience preferred.
