Walsall Council is seeking a Social Care Facilitator to join the ICS team formed between Walsall Health Care NHS Trust and Walsall Council Adult Social Care. The role focuses on supporting hospital discharge, rehabilitation and safeguarding adults while working across partner agencies to improve outcomes, with regular supervision and ongoing professional development.
The ideal candidate will have safeguarding experience, an enhanced DBS check, and demonstrate accurate spoken English.
