We are seeking an enthusiastic and dynamic individual to work within Intermediate Care Services in the delivery of Physiotherapy interventions that will promote real improvements in patient safety, patient experience and clinical effectiveness.

We are looking for a Physiotherapist who has a keen interest in developing a career in a community setting. The successful candidate will support the need for an efficient, proactive service in order to support patients who require rehabilitation following discharge from hospital, or support with hospital avoidance.
Main duties of the jobTo provide an accomplished physiotherapy service working 5 days over a 7-day working week, with the overall aim of providing rehabilitation in the community and avoiding hospital admission.
To provide physiotherapy assessment of patients and to deliver a clinical based, individualised treatment programme in consultation with the physiotherapist as indicated.
To hold responsibility for own caseload working without direct supervision. Supervision takes the form of regular formal training and clinical reasoning sessions, review of cases and the clinical intervention, peer review and case conferences. Advice and support from a senior physiotherapist is available and clinical work is routinely evaluated.
To undertake, with guidance, all aspects of clinical duties as an autonomous practitioner.

To be professionally and legally accountable for all aspects of own work.
To undertake assessment of patients, using clinical reasoning skills and including manual assessment techniques. Assessments may take place within patients homes or within nursing homes.
To formulate and deliver an individual physiotherapy treatment programme based on sound knowledge of evidence based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment techniques and skills, e.g. manual therapy techniques, patient education, exercise classes and other alternative options.
To undertake a defined clinical area of responsibility. To be responsible for providing physiotherapy assessment and treatment plans for patients within this remit.
To formulate realistic goals and participate in discharge planning as appropriate within the multi-disciplinary team. We are an integrated team with social care which may involve working alongside social workers, key workers and reablement officers.
To obtain consent as outlined in Chartered Society of Physiotherapy (CSP), Health and Care Professions Council (HCPC) and Trust policies.
To effectively communicate with patients who may have barriers to understanding or communicating, e.g. blind, deaf, dysphasic, or who do not have English as a first language.
To evaluate patient progress, reassess and alter treatment programmes if required.
To be responsible for maintaining accurate, confidential and comprehensive patient treatment records in line with the CSP, HCPC and to follow Trust documentation policies.
To actively participate in multi-disciplinary team meetings and integrate physiotherapy treatment into the overall treatment programme. This may include discussion of patient care, patient progress and involvement in discharge planning from an acute setting.
