What you will do
  • Engage as an integral member of a multi-disciplinary team, ensuring the needs of vulnerable adults are identified and addressed through comprehensive assessment and care management.
  • Support citizens and carers in their decision-making process, fostering independence and connecting them to community resources tailored to their assessed needs.
  • Play a crucial role in the assessment of mental capacity, ensuring that health, wellbeing, choice, and control are prioritized and respected.
  • Facilitate communication and build strong person-centred relationships with service users and their families or carers, guiding them through their care processes in a compassionate manner.
  • Collaborate with a team from varied disciplines, including partnerships with community agencies, to enable and empower individuals to live independently and exercise autonomy.
  • Participate actively in both individual and team development activities to stay on top of best practices and innovative care approaches.
  • Implement and identify needs for assistive equipment during assessments, referring more complex cases to specialists when necessary.
