What you will do:

– Undertake Occupational Therapy assessments and develop outcome-focused, person-centred plans
– Assess and advise on manual handling requirements, with a strong focus on Optimal Handed Care
– Co-produce care and support plans with service users and families where appropriate
– Provide advice regarding services, equipment, adaptations, and support options
– Deliver and oversee reablement interventions to maximise independence
– Prescribe and arrange specialist equipment and adaptations
– Review and reassess care packages to ensure effective outcomes
– Work collaboratively with multidisciplinary teams and partner agencies
– Support service users, carers, and families through guidance and professional advice
– Maintain accurate case records and utilise electronic recording systems
– Participate in duty functions, team development, and service improvement activities

Yes – HCPC
– Car driver:
Yes – having a vehicle at your disposal helps ensure you can reach service users and collaborate with local partners efficiently.
