What you will do:
- Work collaboratively within a multidisciplinary team to support patient rehabilitation and reablement.

- Deliver high-quality occupational therapy services across the Intermediate Care pathways.
- Liaise with healthcare professionals to ensure holistic patient care.
- Address complex cases by applying your experience to achieve the best patient outcomes.
Qualifications / Requirements:
- Must be HCPC registered.
- Must possess a valid driving licence to effectively travel within the community setting.
