What you will do:

  • Work collaboratively within a multidisciplinary team to support patient rehabilitation and reablement.
  • Assess and develop tailored intervention plans to enhance patient independence in a community setting.
  • Deliver high-quality occupational therapy services across the Intermediate Care pathways.
  • Liaise with healthcare professionals to ensure holistic patient care.
  • Address complex cases by applying your experience to achieve the best patient outcomes.

Qualifications / Requirements:

  • Must be HCPC registered.
  • Must possess a valid driving licence to effectively travel within the community setting.
