We are looking for an enthusiastic, compassionate Practice Nurse looking to make a real difference within a supportive and forward thinking GP practice.

At Little London Surgery we are proud to provide high quality care to almost 9,500 patients in the heart of our community. We are looking for a motivated nurse who enjoys working as part of a friendly multidiciplinary team and is passionate about delivering excellent patient care.

The successful applicant will undertake a range of treatment room duties including chronic disease management, cervical screening, immunisations, wound care, health promotion and supporting patients to manage their long term conditions. Experience in general practice is desirable, although we welcome applications from nurses with transferable skills who are keen to develop within primary care.

Main duties of the job

The successful applicant will work as part of our experienced multidisciplinary team to provide high quality, evidence based nursing care to our practice population.

  • delivering treatment room services including wound care, dressings, injections, ECGs, venepuncture and cervical cytology
  • Undertaking childhood and adult immunisations in line with national guidance
  • Supporting the management of patients with long term conditions including asthma, COPD, diabetes and cardiovascular disease
  • Carrying out NHS health checks and health promotion activities, encouraging patients to make positive lifestyle choices
  • Maintaining accurate, contemporaneous clinical records using EMIS Web and working within practice protocols.
  • Providing safe, effective and compassionate patient-centred care within your scope of practice.
  • Working collaboratively with GPs, Advanced Nurse Practitioners, HCAs, pharmacists and the wider practice team.
  • Participating in clinical audits, quality improvement initiatives and QOF activities to support excellent patient outcomes.
  • Contribution to infection prevention and control, safeguarding, learning events and clinical governance.
  • Maintaining professional registration, mandatory training and continuing professional development in accordance with NMC code,

We are a training practice and members of South 1 Primary Care Network.

We provide ourselves on providing accessible, patient centred care through multidisciplinary team of GPs, Advanced Nurse Practitioners, Practice Nurses, HCAs, Pharmacists and other team members from our South 1 PCN.

Job responsibilities

Provide high-quality nursing care to the practice population, supportinglong-term condition management, preventative healthcare and treatment roomservices in accordance with NMC standards.

Key Responsibilities

Chronic disease management (Diabetes, Asthma, COPD, Hypertension)

Treatment room services including cytology, immunisations, wound care, ECGsand phlebotomy

Health promotion and patient education

Accurate EMIS documentation and coding

Infection prevention and control

Participation in audits, QOF and quality improvement

Maintain CPD, mandatory training and professional registration.

Person Specification Qualifications
  • Registered Nurse with current NMC registration
  • Evidence of continuing professional development
  • Competent in history taking and clinical assessment with scope of practice
