Single Point of Access (SPA)

The Clinical Lead Nurse will coordinate the SPA team, providing triage, assessment and advice for children, young people and families as part of the Healthy Child Programme (HCP) 0‑19. They will use telephone, text and email services to support users and sign‑post to appropriate services.

Key responsibilities
  • Coordinate a team of nurses, assistant practitioners and administrative staff to deliver a high‑quality clinical care programme via the SPA.
  • Provide triage, clinical assessment and advice, and sign‑post to support for children, young people and families accessing the Healthy Child Programme 0‑19.
  • Maintain timely and relevant information to safeguard children and young people, working within local safeguarding policies.
  • Adhere to Trust policies, procedures and protocols, promoting quality patient experience.
  • Maintain accurate nursing records, confidentiality and statistical records.
  • Promote multidisciplinary/multi‑agency working and continuity of care.
  • Utilize service pathways, sign‑post and refer to multi‑agency services and liaise with multi‑agency teams.
  • Oversee daily workload, demonstrating appropriate time‑management.
  • Provide evidence‑based clinical triage and identify poor practice for remedial action.
  • Serve as a professional presence on Trust business, ensuring compliance with policies.
  • Assist senior leadership with quality assurance, governance and audit activities.
  • Develop clinical skills and seek training for future service development.
  • Supervise staff and facilitate continuing professional development.
  • Provide training opportunities for students and learning outcomes.
  • Communicate with empathy and reassurance to children, young people and parents/carers.
  • Ensure patients and service users are involved and participate in service user forums.
  • Maintain a patient‑led focus in all aspects of care and coordination.
Person specification – Qualifications
  • Registered Paediatric Nurse or staff nurse with significant paediatric or public health nursing experience / Qualified Health Visitor or School Nurse.
  • Diploma level education or evidence of post‑registration experience at Band 5 in a community setting (e.g., School Nursing, Health Visiting, Healthy Child Programme).
  • Evidence of personal and professional development.
Skills and aptitudes
  • Initiative and self‑directed working.
  • Effective written and verbal communication.
  • Demonstrated motivation in practice.
  • Monitoring and evaluation skills.
  • Interpersonal skills.
Knowledge and experience
  • In‑depth knowledge of guidance related to the Healthy Child Programme, family‑centred health care, public health agenda.
  • Excellent knowledge of child development (0‑19), including behaviour management, weight management, feeding/nutrition, sleep management, emotional health and wellbeing.
  • Knowledge of NHS policies (e.g., consent, confidentiality, clinical risk management).
  • Significant post‑registration practice within a community setting (School Nursing, Health Visiting, Healthy Child Programme).
  • Experience of multi‑disciplinary / multi‑agency working, including safeguarding, Early Help, SENDi, CAMHS.
  • In‑depth understanding of clinical governance.
