Position: Social Care Facilitator - Ref: SS02626

Social Care Facilitator -
Grade: G7;
Salary Range: £35,412 - £38,220

ICS is a multi‑disciplinary, multi‑agency team operated jointly between Walsall Health Care NHS Trust and Walsall Council Adult Social Care. We are a dynamic, welcoming team working with adults, their carers and families to maximise their potential and promote independence following an acute episode of illness or hospital admission.

Key responsibilities include:

  • Supporting individuals with hospital discharge, rehabilitation, crisis intervention and admission prevention.
  • Undertaking statutory duties in line with the Care Act and Mental Capacity Act.
  • Collaborating with partner agencies to achieve positive outcomes.

Additional responsibilities:

  • Providing regular supervision and peer group support.
  • Participating in ongoing career development and accessing post‑qualifying training.
  • Working weekends and bank holidays on a rotational basis in a seven‑day, 365‑day service.

Qualifications and experience required:

  • Experience of safeguarding adults.
  • Completion of an enhanced Disclosure & Barring Service (DBS) check.
  • Declaration of any overseas residence, study or work and provision of an Overseas Criminal Records Certificate (OCRC) or Statement of Good Conduct (SOGC) if required.

Applicants will be required to have accurate spoken English, as confirmed by the Government’s Code of Practice on the English Language Fluency Duty for public sector workers.
