Walsall Council is seeking a Social Care Facilitator to support adults, carers and families after acute illness or hospital stays. The role involves enabling independence, coordinating discharge, and contributing to community care pathways within a seven-day service framework.
Applicants should have safeguarding experience and be prepared to work weekends/bank holidays on a rotational basis. A DBS check for working with vulnerable groups is required.
