Walsall Council is seeking a Social Care Facilitator to join a multi-disciplinary team supporting vulnerable adults to remain independent and access community resources. You will contribute to assessments, care management and support planning in line with legislation and council policies.
The role requires experience with vulnerable adults, strong communication skills and a good grasp of safeguarding and the Mental Capacity Act.
