Change Grow Live seeks a Prison In-Reach Recovery Coordinator in Walsall. You will assess service users, coordinate resettlement plans from prison, and liaise with agencies to access the right interventions at the right time.
You will advocate for service users and work to reduce substance-misuse harms within the wider community. You will engage with individuals with complex needs, continually prioritising safety, collaboration and person-centred care.
