We’re searching for Registered Nurses that want to work Per Diem or Part Time in the homecare setting, with AM, PM, and weekend shifts available. As a Per Diem RN, you can pick up a lot or a little!

As a part-time RN Case Manager, you will have the same shifts and case load weekly, with flexible options for 2-day, 3-day, and 4-day scheduling!

POSITION SUMMARY:

The Per Diem Registered Nurse (RN) administers skilled nursing care to patients requiring professional nursing services and teaches and supervises the family and other members of the nursing team. These services are performed in accordance with physician’s orders under the direction and supervision of the Clinical Director and Clinical Manager and in compliance with applicable laws and regulation and the policies of the organization.

RESPONSIBILITIES:
  • Seeks and obtains sufficient information necessary to function effectively in the Healthcare setting where assigned.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient’s status by:
    • Reviewing and implementing the plan of care
    • Regularly re-evaluating the patient needs
    • Revising the plan of care as necessary
  • Administers medication and treatments as prescribed by the physician.
  • Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques and maintains communication regarding patient care with the physician, supervisory personnel and other professional staff involved in patient care.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patient condition and care given.
  • Uses supplies and equipment effectively and efficiently and maintains equipment per agency policy.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts towards self-sufficiency and independence.
  • Submits clinical notes within 48 hours of completing the visit.
  • Teaches the patient’s family/caregiver self-care techniques as appropriate; provides medication, diet and other instructions as ordered by the physician and recognizes and utilized opportunities for health counseling with the patients and families/caregivers.
  • Provides and maintains a safe environment for the patient.
  • Identifies and discusses with the clinical/case manager the need for involvement of other members of the health team such as the physical therapy, social worker, etc.
  • Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
  • Assists the patient’s family/caregiver and other health care personnel in providing continuity of care.
  • Performs within the limits of preparations and experience.
  • Accepts responsibility of assignment to perform a specialized procedure, such as IV therapy, only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation continuing education, including mandatory in-service programs offered by the agency.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Performs other duties as assigned.
