Job SummaryThe role of the Heart Failure Nurse within the community has been established to meet the needs of people with heart failure in the Surrey locality. The goal is to optimise management of people with heart failure, reduce unnecessary hospital admissions, and provide a link between GPs and specialist teams. The post holder will offer education and support to health care professionals, patients with heart failure, and those who care for them.
Responsibilities- Communicate highly sensitive information and bad news to patients and carers in an empathetic and supportive manner, empowering them to manage their condition.
- Teach patients and their carers to manage heart failure to the best of their abilities, preventing short‑term and long‑term complications, in one‑to‑one or group sessions.
- Ensure clear understanding of heart failure and its management by communicating effectively with patients and carers; provide and receive complex, sensitive information that requires motivational, empathetic and reassuring skills.
- Advocate for patients, particularly in residential homes, ensuring they receive the understanding and facilities needed to manage heart failure.
- Maintain and develop effective joint working relationships with nursing and medical colleagues throughout partnership working.
- Provide advice and information on heart failure to medical and nursing colleagues and the general public.
- Up to date knowledge of current clinical and professional issues.
- Basic Life Support (BLS) certification.
- Good organisational and time‑management skills.
- Ability to work independently unsupervised.
- Good computer and keyboard skills, including Excel.
- Good communication skills, both written and verbal.
- Auditing experience.
- Presentation and counselling skills.
- Registered General Nurse (RGN) or equivalent HCPC qualification.
- BSc or equivalent qualification in nursing.
- Advanced clinical assessment skills.
- ENB 998 teaching and assessing or equivalent.
- Independent non‑medical prescriber V300.
- Level 7 heart failure specific qualification.
- Qualified as a nurse for 5 years with at least 2 years of cardiac nursing experience.
- Evidence of CPD at Level 7.
Band 7 – £50,008 to £56,908 per year, inclusive of 5% High Cost Area allowance (pro rata).
Contract:
Permanent, Full‑time.
