Diabetes Practice Nurse Job Overview

We are looking for an enthusiastic and experienced Diabetes Practice Nurse to join our friendly, patient-centred team at Hersham Surgery. The role will involve providing high-quality diabetes care, including annual reviews, diabetic foot checks, blood pressure and cardiovascular risk assessment, lifestyle advice, medication support and ongoing management of patients with Type 1 and Type 2 diabetes. You will work closely with our GPs, nursing team and wider multidisciplinary team to support patients in managing their condition and achieving the best possible health outcomes.

We are looking for someone who:

Is a Registered Nurse with relevant diabetes experience/qualification
Has excellent clinical and communication skills
Is confident working autonomously within their scope of practice
Is passionate about patient education and preventative care
Works well as part of a friendly and supportive team

Responsibilities

Annual reviews
Diabetic foot checks
Blood pressure and cardiovascular risk assessment

Medication support
