The Purbeck PCN Pharmacy Team includes Clinical Pharmacists, Pharmacy Technicians and a Care Co-ordinator, working alongside General Practice and the PCN clinical teams. This is a full time post of 37.5 hours a week but part time of a minimum of 24 hours over 3 days would be considered. The Clinical Pharmacist will be required to work flexibly to meet the needs of the service and this role will include supporting patients in care homes and also home visits for housebound patients.

The Clinical Pharmacist will need to liaise with GP practices, hospital pharmacy teams, local community pharmacists,social services, care homes and care agencies to ensure any continuingmedicine management issues are addressed and managed throughout the patient scare pathway. The Clinical Pharmacist will provide relevant information to patients/carers and staff on matters relating to medication. The Clinical Pharmacist will counsel patients onhow to take their medicines and identify any barriers to them taking their medication.

The Clinical Pharmacist will undertake face to face and person-centred consultations with the emphasis onshared decision making. This post will involve assisting the PCN in achieving national requirements, NICE implementation, and utilisation of medicine optimisation initiatives.

Job responsibilities

Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly,people in care homes, those with multiple co-morbidities and people with learning disabilities or autism.

Review the on-going need for eachmedication, ensuring monitoring needs are performed and use the opportunity to support patients achieve better health outcomes through shared decision-making conversations

Support patients to take their medicationsto get the best from them, reduce waste and promote self-care

Make appropriate recommendations toprescribers

Promote safe disposal of medicines and recycling of inhaler

Patient facing / video consultation Long-term Condition clinics

Help to set up pharmacist-run long termcondition clinics particularly for patients with complicated medicationregimes

Optimisation of blood pressure control utilising BP@Home.

Patient facing / video consultation Care Home Medication Reviews

Undertake clinical medication reviews,formulate a medicines optimisation plan jointly with the patient/ carer and facilitate its implementation by working with other professionals, service sand agencies as necessary

Support patients to wean from medicationsincluding opioids, gabapentinoids and antidepressants in line with national guidance.

Work with care home staff to improve safety of medicines ordering and administration.

Take part in the pharmacy team centralrota to answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients.

Suggest and recommend solutions using clinical knowledge and trusted pharmaceutical sources

Carry out medication reviews and reconciliation for all newpatients and for patients whose care is transferred back into primary care.

Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

Support the Pharmacy team in to manage blood monitoring ofHigh-Risk Drugs and DOACs across the PCN.

Provide specialist expertise in the use of medicines whilst helping to address both the public health and social careneeds of patients at the PCNs practices and to help in tackling inequalities

Undertake and lead on any specific medicines optimisation-basedprojects/portfolio areas of work as may be required by the Pharmacy Team,e.g., diabetes medication, blood pressure optimisation, lipid loweringtherapy

Collaborating Working Relationships

Having a leadership role in supportingfurther integration of general practice with the wider healthcare teams(including community and hospital pharmacy) to help improve patient outcomes,ensure better access to healthcare and help manage general practice workload

Work collaboratively with ICB medicinesmanagement teams

Develop relationships and work closely with other pharmacy professionals within and across the PCN and the widerhealth and social care system
