Practice Clinical Pharmacist

The Avenue Surgery is seeking an enthusiastic and forward-thinking clinical pharmacist to join our like minded team. The successful individual will play an important role in working with the current pharmacy team to ensure excellent standards of repeat prescribing, medication reviews, medicine optimisation and contributing to improving the quality of care to our practice patient population. The pharmacist will be in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.

The individual will be required to enrol and complete the CPPE pathway and will be supported to develop their role to become a non-medical prescriber(if not already qualified).There could a scope for future management opportunities within the role.

• Patient facing clinics dealing with LTC, clinical medication review, care home medication and domiciliary medication reviews.
• Management of common/minor/self limiting ailments and patient medicines support.
• Management of medication at discharge for hospital, review of unplanned hospital admissions.
• Repeat prescribing, risk stratification and service development and medicines safety.
• Ensuring that all local and national guidelines are implemented and formulary recommendations are followed.

The Avenue Surgery is a non-dispensing, training practice, caring for circ.
18500 patients in and around the semi-rural town of Warminster, Wiltshire which is within an easy commute to the cities of Bath and Salisbury. The practice is led by 7 GP partners and a hard working team of both clinical and administration staff. The team are very committed to the surgery vision to provide a service that works with, and for, the community to meet the widest range of care, health promotion and education for our patients and staff members.

• Have experience and an awareness of common acute and long term conditions that are likely to be seen in general practice.
• Experience at a high level of competency with computers.
• Experience with working as part of a multi-disciplinary team.

• Has previous experience working within a GP environment.
• Has working knowledge of TPP Systemone computer system.

• Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
• Enroll and complete the CPPE PCPEP pathway.
• Minimum of 2 years experience as a pharmacist, demonstrating within a practice portfolio.
• May hold or be willing to work towards an independent prescribing qualification.
