Role
Summary:
To provide clinical care as an Enhanced Clinical
Practitioner (ECP) in line with the standards of practice and code of contact
of their registering body. The post holder will undertake autonomous clinical
practice ensure safe, effective, and high-quality patient care.
Main duties of the job
Provide specialised clinical assessment, examination, and management
of patients presenting with acute and undifferentiated conditions in line with
their enhanced training and scope of practice.
Triage and
prioritise patients appropriately, delivering same-day and urgent care services.
Independently assess, diagnose and manage a wide range of minor
illness presentations.
Support in the
management of chronic and complex conditions which fall with their enhance
scope of practice and training.
Identify red flags
and escalate or refer patients appropriately to secondary or urgent care
services.
Request, interpret,
and act on diagnostic investigationse.g.,
bloods, ECGs, imaging where appropriate.
Prescribe safely as
an independent prescriber,if
qualified, in line with local and national guidelines.
Provide clinical decision-making in complex and uncertain situations,
recognising limitations and seeking senior support where needed.
Deliver patient
education, safety-netting, and health promotion advice.
Maintain accurate,
contemporaneous, and legally compliant clinical records.
Work collaboratively
within the multidisciplinary team, supporting
patient flow and demand management.
Undertake risk
assessment and management, including safeguarding concerns and deteriorating
patients.
About us The Avenue Surgery is a non-dispensing, training practice, caring for circ.
18000 patients in and around the semi-rural town of Warminster, Wiltshire which is within an easy commute to the cities of Bath and Salisbury.
The practice is led by 6 GP partners and 4 salaried GP's supported by a hard working team of both clinical and administration staff including a specialist Living well team who support our most vulnerable and older community.
A strong prescribing team lead by a pharmacist and a pharmacist, 2 pharmacy technicians and 3 prescription clerks.
The team are very committed to the surgery vision to provide a service that works with, and for, the community to meet the widest range of care, health promotion and education for our patients and staff members.
We are a friendly, dedicated team with the determination to provide high quality clinical care for our patients.
