We are seeking a compassionate and skilled Hospice Registered Nurse (RN) to provide high‑quality, patient‑centered care to individuals with life‑limiting illnesses. This role focuses on promoting comfort, dignity, and quality of life for patients and supporting their families. The Hospice RN works collaboratively with the Interdisciplinary Team (IDT) and complies with all applicable federal, Georgia state, and organizational regulations.
Key Responsibilities- Coordinate and manage patient schedules to ensure timely and appropriate care
- Perform comprehensive assessments of patients’ physical, emotional, and psychosocial needs
- Develop, implement, and update individualized plans of care in collaboration with the IDT
- Provide symptom management, including pain and comfort measures
- Educate patients and families on disease progression, end‑of‑life care, and available resources
- Provide emotional support to patients and families, including education on bereavement services
- Maintain accurate, timely, and compliant clinical documentation
- Participate in interdisciplinary team meetings and care conferences
- Participate in the second on‑call rotation (typically every six weeks), which may include weekends and holidays; schedules may change based on patient needs
- Active, unrestricted Registered Nurse (RN) license in the State of Georgia
- Minimum of one (1) year of nursing experience (hospice experience preferred)
- Current CPR certification
- Valid driver’s license and reliable transportation
- Strong communication, organizational, and interpersonal skills
- Ability to provide compassionate, patient‑centered care in home and facility settings
- Ability to safely lift, transfer, reposition, and assist patients of varying weights, consistent with the physical demands of the role, with or without reasonable accommodation
- Monday through Friday, 8:30 a.m. to 5:00 p.m.
- Participation in second‑call rotation as assigned
