As a Male LPN with Modani Care, you will play a vital role in providing individualized care to our clients, focusing on their physical, emotional, and mental well-being. You will work closely with our care team to develop and implement personalized care plans, ensuring that our clients receive the best possible care in the comfort of their own homes. Your exceptional communication skills will enable you to build strong relationships with our clients, their families, and other healthcare professionals.

• Provide high-quality, individualized care to clients, including administration of medications, wound care, and other medical treatments

• Develop and implement personalized care plans in collaboration with the care team

• Conduct thorough assessments of clients' physical, emotional, and mental health

• Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care coordination

• Maintain accurate and detailed records of client care and services provided

• Participate in on-going education and training to stay current with industry developments and best practices

• Current LPN license in the State of Georgia

• Ability to work varied shifts, including days, evenings, and weekends

• Valid driver's license and reliable transportation

• Strong communication and interpersonal skills
