Job Summary

The Service Coordinator assumes responsibility for coordinating programs and services to help residents maintain a good quality of life and age in place. The role includes assessing and identifying resident and family needs; gathering outside resources; networking with community organizations; conducting psychosocial assessments; case management; maintaining resident files; coordinating services and care plans; submitting performance reports to HUD; engaging residents; organizing programs;

and coordinating home care services.

Responsibilities
  • Coordinate programs and services to help residents maintain a good quality of life and age in place.
  • Assess and identify resident and family needs and outside resources.
  • Assist with physical, mental, psychological, and social aging needs.
  • Network with community organizations and seek affordable and new services.
  • Initiate and complete psychosocial assessments, interviewing patients, families, and others.
  • Provide general case management and referral services; maintain complete, updated resident files.
  • Coordinate services for optimal living (i.e., aging in place and wellness philosophies).
  • Complete Care Plans/Quarterly Monitoring with residents needing referrals; follow up on all services at and after implementation.
  • Submit semi-annual performance reports to HUD to maintain funding.
  • Engage all residents to identify areas of need and make necessary referrals; organize and promote resident involvement in on/off-site programs.
  • Assist in coordinating home care services to enable independent living and aging in place; develop a resource directory of service providers; continually educate residents and staff on matters including service availability, application procedures, resident rights, health/safety/fitness, and aging issues.
  • Coordinate counseling, occasional nurse visits, preventative health screening/wellness, and legal advocacy.
Education
  • Minimum associate's degree or equivalent in social work, psychology, or gerontology preferred.
Experience
  • Two or more years of experience in social service delivery with the elderly and/or family populations is a plus.
  • Demonstrated knowledge of supportive services and other resources in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for residents served.
Skills / Abilities
  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and resident software data systems (AASC online, Care Guide, Real Page, One Site, or similar related systems).
