Responsibilities

• Provide specialized nursing care to patients with complex, chronic conditions in a long-term acute care setting.
• Assess patient health status, develop and implement individualized care plans.
• Administer medications, treatments, and monitor patient responses.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Educate patients and families on health management and discharge planning.
