Licensed Practical Nurse – Care Manager

Advisa Care is seeking an experienced, motivated, and reliable Licensed Practical Nurse to serve as Care Manager at our Independent Living Community in Warren, MI. This full‑time, salaried position offers excellent benefits and the opportunity to utilize nursing and management skills.

• Hiring, training, and supervising staff – Team Lead, Med Techs, Home Health Aides.
• Building relationships with staff, residents, families, and community directors.
• Conducting assessments of new residents or those being discharged from a hospital or rehab center.
• Being on‑call and overseeing all shifts.
• Working the floor as support.
• Handling weekly payroll and scheduling.

• Utilizing all nursing and management skills.
• Providing exceptional care to a diverse patient population.
• Collaborating with an interdisciplinary team of care providers.
• Private duty, skilled, and hospice care experience.

• Current Michigan LPN license.
• Minimum one year of experience.
• Updated TB test and current CPR certification.
• Clear criminal history/drug screen.
• Critical thinking, task competence, and ability to think on the fly.
• Problem‑solving skills and ability to multitask.
