Rapidly growing! Urgently hiring! $1,500 sign‑on bonus!!! Homecare with Heart offers the most competitive pay scale in the area. We’re looking for compassionate and experienced LPN to provide care for our clients in their homes. Pay for LPN
: $26‑28 per hour depending on experience and availability. This position serves Trumbull County.

• Administer medication in accordance with standards of practice.
• Encourage social rehabilitative skills and promote self‑care and independence.
• Perform client treatments in accordance with Plan of Care.
• Assist the consumer in learning appropriate self‑care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document, and report any change in client condition to Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in‑services, orientation and staff meetings.
• Provide service in accordance with agency’s philosophy, standards, policies and scope of practice. Participate in Performance Improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

• Valid State LPN License.
• Valid driver’s license; use of automobile required due to travel involved.
• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
• Ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Proficient with computer, laptop or tablet.
• Independent thinker; able to recognize potential problems, gather information, and know when to contact a supervisor.
• Promotion of excellent customer service and exceptional client care, while staying within agency rules and rules of each program served.
• Oasis experience required.
