Occupational Therapist, Lorain/Western Cuyahoga Counties - ( 25000
BAL )
- Organizes and conducts Occupational Therapy program to facilitate the rehabilitation of people with physical and/or psychosocial disabilities.
- The Occupational Therapist directs the patient's participation in selected activities/treatments to restore, re-enforce and correct pathology to promote and/or maintain good health.
- Performs evaluation(s) and develops an effective treatment plan appropriate to a patient’s rehabilitation needs within the care continuum.
- Educates patient/family/caregiver in applicable health information, treatment and prevention.
- Initiates and participates in the discharge planning process.
- Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.
- Performs other duties as assigned.
- Complies with all policies and standards.
- For specific duties and responsibilities, refer to documentation provided by the department during orientation.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- This position is in Home Care Services.
- This position is part time 20 hours per week.
- Monday-Friday.
- 100% travel to the patients homes to provide care.
- The shift will be 8:00 AM to 5:00 PM.
- The territory will be but not limited to:
Lorain and Western Cuyahoga counties and surrounding areas. - The patient population is pediatrics.
- Graduate of an accredited Occupational Therapy program required.
- Evaluation and treatment skills necessary to provide Occupational Therapy appropriate to the developmental and chronological age of patients on caseload.
- Professional initiative, objectivity, good time management and interpersonal skills.
- Physically, cognitively and perceptually able to provide patient treatment and able to assist in physical mobility of patients and equipment.
Basic Life Support (BLS) Required within 90 Days. - Is awaiting Ohio licensure status of Limited Permit required.
- Licensed by the State of Ohio to practice Occupational Therapy required.
