Cleveland, OH (In‑Person/Field‑Based)

Schedule:

Monday–Friday, 40 hours/week

Hourly Rate: $18.00 – $25.00 per hour

Position Summary: Motivated and Empowered Inc. is seeking a compassionate, mission‑driven Full‑Time Case Manager to provide direct support and case coordination to youth and families experiencing systemic barriers including homelessness, poverty, violence, foster care involvement, and behavioral health challenges. The ideal candidate will play a key role in the stabilization, empowerment, and long‑term success of those we serve by linking them with internal programs and external resources to meet their educational, emotional, financial, and social goals.

Key Responsibilities
  • Conduct comprehensive intake assessments to identify strengths, needs, barriers, and goals.
  • Develop individualized service plans (ISPs) with measurable outcomes in collaboration with clients.
  • Provide short‑ and long‑term case management, including crisis intervention and referrals to housing, mental health, education, and workforce development programs.
  • Track client progress and adjust service plans as needed; maintain consistent contact (in‑person, virtual, or phone) on a weekly or biweekly basis.
  • Advocate for youth and families in educational settings, court hearings, and service agency meetings as needed.
  • Maintain an updated directory of community resources, including behavioral health, housing, food assistance, and academic support.
  • Coordinate warm hand‑offs and follow‑up on referrals to ensure successful connection to services.
  • Engage in collaborative case conferencing with internal teams and external partners (schools, probation, child welfare, etc.).
  • Maintain accurate, timely, and confidential case notes in accordance with agency and regulatory standards.
  • Complete required reporting, evaluations, service plans, incident reports, and follow‑up documentation in our case management system.
  • Participate in internal audits and ensure compliance with state and local reporting requirements.
  • Attend weekly team meetings, professional development sessions, and supervisory check‑ins.
  • Assist with programmatic events such as family workshops, youth retreats, and seasonal giveaways.
  • Contribute to the creation of culturally competent, trauma‑informed environments for all youth and families served.
Required Qualifications
  • Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, Family Studies, or a related field.
  • 1–2 years of experience working with vulnerable youth and families in a human/social services or nonprofit setting.
  • Strong knowledge of community resources and systems (child welfare, juvenile justice, education, housing, etc.).
  • Demonstrated ability to maintain professional boundaries and handle sensitive situations with empathy and confidentiality.
  • Excellent communication, organizational, and documentation skills.
  • Proficiency in Microsoft Office Suite and willingness to learn agency software systems.
Preferred Qualifications
  • Case management or outreach experience with youth ages 12–24 and/or young parents.
  • Familiarity with trauma‑informed care, strengths‑based approaches, and motivational interviewing.
  • Bilingual in English and Spanish or another language commonly spoken in our service area.
  • Reliable transportation and willingness to travel within Cuyahoga and Summit Counties.
  • Be punctual, dependable, and responsive to the needs of clients and team members.
  • Demonstrate cultural humility and commitment to serving diverse populations with dignity and respect.
