Overview
Licensed Practical Nurse (LPN) – Ambulatory at UVA Health Northern VA & Culpeper. This role provides patient-centered care under the direction of a RN/LIP and Clinic Leader, serving all age populations with appropriate consideration of developmental needs. A $5,000 Sign-On Bonus is offered.

Responsibilities- Patient Care:
Provide assessment, evaluation, and management of patients; participate in collection of patient data and determine nursing care needs by recognizing relationships between data and health status; identify resources as needed. - Communication:
Share information with others involved in patient care; engage with patients and chosen support systems to identify individualized care plans; use SBARQ for interdisciplinary communication and handoffs; document information promptly. - Teamwork:
Collaborate with the interdisciplinary team to plan and modify care plans based on patient needs and conditions. - Medical Knowledge:
Apply core knowledge of biomedical and clinical sciences to patient care; identify nursing interventions and goals; seek opportunities for process improvement and professional development; integrate knowledge into practice.
The incumbent may be asked to perform additional duties as assigned.
QualificationsEducation: High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of nursing program approved by the State Board of Nursing.
Experience: 1 year of relevant experience required. Refer to Life Support Training Policy for details.
Licensure: Applicable state LPN license required.
Additional Skills/Requirements: Ability to complete general and department-specific skills validation and competency testing; ability to interpret information to determine patient care needs by age; ability to adjust care accordingly.
Physical Demands: Physical Demand Code: 20A, Nursing. Work Function/Activity:
Medium to Heavy Physical Demand. Requires standing, walking, bending/stooping; strong communicative, auditory, and visual skills; ability to lift/push/pull up to 100 lbs with Safe Handling Equipment as needed.
Other: May require safety equipment (e.g., HEPA mask) for infection prevention. On-call responsibilities as directed. Ability to travel between campus buildings, remote facilities, and out-of-town as needed (No for this role).
