Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient and family care at home or in alternative settings. You will perform comprehensive RN assessments, develop care plans, and coordinate with an interdisciplinary team to deliver skilled nursing, symptom management, and education.
You will communicate changes in patient condition, document thoroughly, and support quality initiatives. A valid RN license and hospice/home health experience are required.
