Community Integrated Care is currently seeking a dedicated and compassionate health and social care professional for a Service Manager role based in Burtonwood, Warrington.
This is an exciting opportunity for a skilled and experienced leader, as you will lead a team of one assistant manager, Advanced Support Workers, supporting one young man with a learning disability, Tourette’s, OCD and complex needs, to lead the Best Lives Bolder
.
- Salary starting from £42,000 per year

As Service Manager, you will lead, inspire, and develop a team of Advanced Support Worker Specialists, ensuring they feel supported, motivated, and confident in their roles. You will deliver effective practice leadership and ensure the consistent implementation of personalised support plans, promoting a positive, ambitious, and person‑centred culture within the service. You will work closely with the NHS HOPE(s) Team and other professionals to uphold quality, safeguarding, and best practice, while ensuring robust care planning, risk management, and record keeping.
Your leadership will drive high standards, accountability, and continuous improvement, helping the young man reach his potential.
Practice leadership is central to this role. You will support the team in implementing the care and support plan, model best practice, and ensure that proactive, tailored interventions are delivered consistently. You will facilitate positive risk‑taking, encourage reflective practice, and promote continuous learning, ensuring the team is skilled, confident, and resilient.
Working in Partnership with the NHS National HOPE(s) TeamAs Service Manager, you will work closely with the NHS National HOPE(s) Team, who provide clinical leadership for the care package. This partnership brings together clinical expertise and social care leadership, creating an integrated model of support. The HOPE(s) Team ensures that the support delivered is therapeutically informed, structured, and responsive to changing needs.
About the Young Man and the TeamSince his discharge from long‑term hospital care 18 months ago, the young man has made remarkable progress. He has a complex profile, including a diagnosis of Autism, Learning Disabilities, Tourette’s Syndrome, OCD, Receptive Language Disorder, a PDA (Pathological Demand Avoidance) profile, and extreme trauma‑related anxiety. The team at Burtonwood are highly experienced in supporting people with autism and complex behaviours, maintaining a structured, nurturing environment that promotes wellbeing, confidence, and meaningful engagement.

You will be an experienced leader in complex care, autism, or supporting people with complex behaviours, with a track record of developing high‑performing teams. You will be confident in managing behaviours that may challenge, using proactive, reflective, and innovative approaches. Most importantly, you will be passionate about delivering exceptional, life‑changing care and committed to putting time and energy into this service.
QualificationsLevel 5 in PBS (Positive Behaviour Support) or willingness to work towards one.
Professional qualification in Health and Social Care (e.g., Nurse, Social Worker, SVQ Level 3 plus supervisory units (Scotland) / Care Management Level 3 (England)).
Comfortable using basic IT systems required within the role – training can be offered to support this area.
