What you will do:

• Collaborate with a multidisciplinary team to support adults in regaining their independence and preventing hospital readmissions.
• Develop tailored care plans and provide systematic assessments to cater to individuals in times of crisis or recovery.
• Engage in providing technology-enabled care and facilitate the provision of required equipment and adaptations for individuals at home.
• Monitor, review, and evaluate care plans to ensure they align with clients’ changing needs and promote ongoing recovery and rehabilitation.
• Liaise with healthcare professionals, families, and external agencies to coordinate a seamless care experience.
