Health Care Assistant

We are looking to recruit an enthusiastic Health Care Assistant who has a genuine interest in providing safe and effective care for patients  successful candidate must have demonstratable experience, a wide range of clinical skills and will be confident to work without direct supervision. Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change.

Main duties of the job

The role requires the staff member to work as part of a Community Nursing Service to deliver high-quality care to patients in their own homes in accordance with individual care plans. Provide personal and palliative care, ensuring patients are treated with dignity, respect, and compassion while supporting their comfort and wellbeing. Demonstrate the ability to work independently and autonomously within the community setting, effectively manage responsibilities while maintaining a high standard of care.

Support the Community Nursing Team by managing the ordering and organisation of clinical supplies and stationery to ensure services operated smoothly. Consistently follow Trust policies, procedures, and professional standards in all aspects of work while maintaining strict adherence to health and safety regulations, infection prevention, and control guidelines. Ensure patient confidentiality is always maintained and information is handled sensitively and responsibly in line with legal, ethical, and organisational requirements.

Contribute to a safe, supportive, and professional care environment for patients receiving community-based nursing services.

Job responsibilities

Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on the 1st April 2026. As such, if your start date is on or after 1st April 2026, you will be employed by the newly formed organisation. If you join Bridgewater prior to this date, your employment will automatically transfer into the new organisation.

Job Purpose:

To assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting.

Duties and Responsibilities:

To work as part of an integrated Community Nursing Service in providing specific care to a patient in the community setting as written in the care plan.

To provide personal care and support to palliative patients in their own homes.

To work alone and unsupervised and the ability to work autonomously.

To be responsible for the ordering of clinical supplies and stationery for the Community Nursing Team.

To work in accordance with Trust Policies and Procedures.

To maintain high standards of personal hygiene adhering to Health and Safety Guidelines and Infection Control Guidelines within the trust.

To respect confidential information during the course of your work and refrain from discussing such information without consent of the patient/client or a person entitled to act on his/her behalf, except where disclosure is required by law or by the order of a court or is necessary in the public interest.

Clinical

To monitor skin integrity of patients at risk of pressure ulcers formation and provide basic preventative information.

To provide full personal care to patients in the terminal phase of life.
