We are looking to recruit a new post of Quality Assurance and Reducing Mortality Lead in our Pathways to Recovery in Warrington
. The successful candidate will be an integral part of our leadership team helping us reduce mortality rates amongst the service user populace through analysis and learning while driving improvements in service quality, and helping us to comply with the Care Quality Commission, and regulated activities.

Hours:

37.5 hours per week

Full Time Salary Range: £35,220.59 - £37,184.91 (dependent on experience, pro rata for part time)

  • Please note Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About the role
  • Lead on carrying out internal investigations and external report writing following deaths in service, primarily notifications to CQC, and coronial reports.
  • Embed the CGL learning culture by sharing lessons from all incidents including deaths and working with staff to reduce recurrence.
  • Promote a culture of learning by coaching staff to give and receive constructive feedback to enhance service delivery and user experience, and the design and delivery of training and workshops.
  • Approach presenting issues systemically, considering factors such as mindset, behaviour, systems, structures, and long‑term patterns.
  • Conduct regular audits of service delivery, highlighting best practice, implementing quality improvement plans, and evaluating their impact.
  • Stay up to date with internal CGL and sector‑wide quality improvements, emerging evidence‑based practice, and relevant legislation and regulation.
  • Working closely with our service user involvement lead helping to analyse service user feedback, complaints, and compliments to identify themes and drive service improvements.
About you Education, Knowledge and Experience
  • Experience with in Social Care and Drug & Alcohol services, with a strong understanding of recovery‑focused practice.
  • Demonstrated capability in project development, change management, quality systems, audits, and service improvement.
  • Skilled in serious incident investigation, including root cause analysis and learning dissemination.
  • Knowledgeable in regulated service environments, including CQC requirements and Key Lines of Enquiry.
  • Confident in delivering training and coaching using engaging and inclusive approaches.
  • Strong understanding of safeguarding for children and vulnerable adults.
  • Proven ability to analyse complex data sets to inform conclusions, priorities, and improvement actions.
  • Effective at influencing stakeholders at all levels and leading teams through change while modelling organisational values.

  • Excellent communication skills, both written and verbal, across a range of stakeholders including service users, staff, and partners.
  • Able to work independently and under pressure, with well‑developed organisational and time management abilities.
  • Demonstrates empathy, professionalism, and non‑judgement in all interactions.
