Childrens Community Nurse

We are looking for a RSCN / RN Child Nurse to join our Children’s Community Nursing Team delivering care to children within a range of community settings, including family homes, schools, nurseries and clinics. The post will involve providing nursing care for children and young people with complex health needs, acute and long-term conditions. You should be able to demonstrate relevant and current post‑registration experience of care delivery to children/families.
The ideal candidate will possess good communication and organisational skills and have the ability to work both independently and as part of a team.
As a Children’s Community Nurse, you will provide specialist nursing care to children and young people with acute, complex, chronic, life‑limiting, or additional health needs in a range of community settings, including the child's home, school, and community clinic. The role involves assessing health needs, planning and delivering evidence‑based care, supporting hospital discharge and working in partnership with children, families, schools, and the wider multidisciplinary team.
The Children’s Community Nurse provides education and training to parents, carers, and school staff and ensures that care is child‑centred, safe, and responsive to the child’s developmental, emotional, and social needs.
Job Purpose: To work within the Children’s Community Nursing Service, delivering high quality, evidence‑based nursing care for children and young people with short‑ and long‑term conditions.
- Identify the health needs of children to be nursed in the community and liaise with multi‑agency partners to promote collaboration and effective communication.
- Work across the Children’s Community Nursing Service developing skills in complex, acute and long‑term conditions care.
- Support children with life‑limiting and life‑threatening conditions, prevent hospital admission and facilitate early discharge.
- Maintain strong links with General Practitioners, Health Visitors and School Nurses.
- Assist in the ongoing development and management of the Children’s Community Nursing Service.
- Accept responsibility for own professional practice as a reflective, accountable clinical practitioner.
Clinical Management and Care Delivery
- Receive referrals into the service and ensure robust assessment, planning, implementation, and evaluation of programmes of care that are family‑centred.
- Deliver high quality nursing care, health promotion and education that contributes to well‑being and illness prevention, ensuring all practice is evidence‑based.
- Ensure care is in accordance with Trust‑agreed clinical standards, policies and guidelines, and that it is provided in a competent manner with appropriate supervision when necessary.
- Possess up‑to‑date clinical skills to closely monitor the child's progress, detect deterioration promptly, and follow agreed care pathways.
- Provide accurate updates to senior staff on the child's physical and emotional wellbeing, and report any deterioration promptly.
- Undertake clinical tasks requiring advanced skills, e.g. administration of IV antibiotics, ensuring informed consent is obtained prior to any procedure.
- Participate in supervision and support of junior staff, delegating and prioritising duties, tasks and workload.
- Contribute to efficient budget management by optimising resources and reporting anticipated budgetary needs or pressures.
- Participate in the discharge process, both to and from the service, ensuring appropriate referral to other healthcare staff or agencies.
- Provide accurate up‑to‑date activity data related to service provision.
Communication and Organisational Awareness
- Promote good communication links between primary, secondary and tertiary services, fostering seamless care.
- Attend and participate in multidisciplinary meetings, safeguarding procedures and case conferences as required.
- Be aware of and participate in risk assessment for the benefit of self, colleagues and clients, in line with Trust policies, particularly the Lone Worker Policy.
