An exciting opportunity has arisen to join the Integrated Medicine and Community Clinical Business Unit as a permanent Band 7 Ward Manager working closely with the Matron and Lead Nurse to provide safe, sustainable, excellent care on Ward A9. Ward A9 is a 34 bedded elderly & general medical ward.

Under the guidance of the Matron the successful applicant will contribute to delivering managerial and professional support on a day to day basis to all nursing staff, ensure the smooth running of the ward and provide excellent clinical care, whilst also supporting the delivery of key national performance metrics.

We are looking for an enthusiastic, dynamic, and innovative individual who is passionate about leading a team who provide high standard evidenced based care for our patients. You will lead the ward on all aspects of clinical care and be focused on patient safety, patient experience and quality assurance frameworks. You must be able to demonstrate a compassionate approach to the care of patients and the public, whilst also possessing the ability to ensure robust clinical operational demands are met.

Main duties of the job

CLINICAL RESPONSIBILITIES

  • Act autonomously as an expert clinical practitioner in frailty care
  • Provide advanced assessment, diagnosis, treatment and management of complex patients
  • Lead Comprehensive Geriatric Assessment (CGA) and individualised care planning
  • Manage a complex and unpredictable caseload, including multimorbidity, frailty syndromes and diagnostic uncertainty
  • Independently request, interpret and act on investigations
  • Practise as an Independent Prescriber, including medication optimisation and deprescribing
  • Act as a senior clinical decision-maker regarding admission, discharge and risk management
  • Lead multidisciplinary ward rounds and clinical reviews
  • Support admission avoidance through ambulatory frailty pathways, SDEC and virtual ward models
  • Lead advance care planning and complex communication with patients and families
  • Provide expert advice, second opinions and consultancy across the Trust and wider system
  • Develop and implement evidence-based pathways, protocols and standards
  • Incorporate new evidence, technologies and innovations into clinical practice
  • Challenge traditional models of care and lead development of new approaches
  • Maintain advanced clinical competencies and ensure safe, high-quality care
  • Provide clinical oversight as part of Frailty Virtual Ward MDT including Out of Hours
