What you will do:

• Screen and triage social care referrals, ensuring prompt and appropriate responses.
• Manage short‑term caseloads effectively and conduct Care Act 2014 assessments to determine eligibility.
• Complete Mental Capacity assessments and undertake Best Interest decisions.
• Conduct safeguarding screenings and S42 enquiries, addressing concerns quickly and effectively.
• Engage in Continuing Health Care processes and complete carers assessments.
• Work collaboratively with individuals, families, carers, and partner agencies to provide a holistic and strengths‑based social work service.
• Apply key legislation such as the Care Act 2014, Mental Health Act, and Mental Capacity Act to your daily practice.
